Full context of this passage, as far as I'm aware, is that a woman is making fun of these two rabbis for being morbidly obese and says there's no way their wives are having sex with them at that weight. In response, the rabbis tell her that (1) their wives are even fatter and (2) a man's genitalia is proportionate to his belly. Hence the discussion of exactly how proportionate that might be.
